Is 'unlimited' or 'unmetered' bandwidth in hosting plans truly limitless?

There's no such thing as unlimited data/bandwidth as someone is paying for it.

Some use a tiny amount and their under usage gets shared around the data centre to help balance the traffic

Everything has limits. The computer has a limit on how much bandwidth it can use, the wire that comes to the computer, the space on the computer and other things influence limits.

It would depend, there is always a limit somewhere, it could be an actual bandwidth limit, they will drop the speed but still allow unlimited.

Not all bandwidth costs the same so sometimes it might be low enough of absorb, then you have to also take into account that not everyone will use massive amounts of bandwidth.
