When does a site require dedicated servers?

Dedicated servers are great for sites with a lot of visitors that uses a lot of bandwidth.

Dedicated servers do not share resources with others sites; thus your site will be able to perform to its full potential.
 
There may be security issues too. I know of at least one site that was removed to a dedicated server because it kept getting hacked and bringing down the server. It wasn't such a problem for the site itself, but was unfair on the other clients on the shared server.
 
Upgrading to a dedicated server should be the answer if a VPS does not fit your requirements anymore. You will have more resources assigned to you and there will be no restrictions as you are no longer sharing a server with anyone else.

Shared hosting has restrictions and many companies take action if the server is being affected by a busy site. This busy site would then require an upgrade.

Shared hosting for example is really not suited for sites that require a lot of resources - such as streaming which can be very resource intensive. Also dedicated servers sometimes come with full root access so you have complete control over your own server (You should be able to request for root access if it is not provided).
 
I believe you should move to dedicated server when you have outgrown a VPS hosting solution. Generally, I suggest a path of Shared -> VPS -> Dedicated to the customers and this can be known when the application performance starts degrading at any of the above levels.

The freedom of a VPS is good but with the current technology VPS' fail to provide the "guaranteed" system resources properly. Another resource monster VPS in your VPS node can easily take your server to a crawl. A dedicated is therefore still the only guaranteed solution for guaranteed system resources.
