You'd be surprised just how quickly you can catch a second wind in the middle of the afternoon. I just completed a seminar in Atlanta over the weekend - Thursday-Monday and I THINK I got 2 hours sleep each night. Seminars from 7am-8pm, networking until 2am, catching up with work etc until 5 or 6am.
You have to remember that when you're pulling long shifts, or days awake, it's not like you're just sitting there doing nothing, you're actually working. I have the fortunate habit of being able to tell my body to sleep, and within 5 minutes, I'll be passed out, but 15 minutes I'm wide awake and ready for another 8 hours without any problem.
And yes, the 24-48 hours used to scare a few people. I became somewhat of a sideshow for one of my sisters years ago when she'd bring friends by the house etc.
My friends hate it when there's a weekend planned somewhere as by 2am they're ready to head to bed, and I'm still up and running It's definitely not for everyone (staying up all the time), but if you are productive and can remain on task, then it's a wonderful benefit!
