Colocation Questionaire

I am looking for a questionaire or survey that I could send to several colocation or hosting companies. Our IT group would like to get some quotes as we are contemplating moving our 1800 square foot data center. We have our own IT folks so a full suite of managed services is not necessary.
I am still getting the full specs of our servers etc but I thought I would see if anyone has a good survey that will allow me to compare apples to apples.

You might want to check out Microsoft Offices website for some templates that you could edit to make into a Colocation checklist. Usually your best bet is to haul yourself down to the DC if within proximity and see what they have to offer and ask the questions yourself.
